As part of our commitment to continuous improvement, Sterling IRB’s informed consent form version format has changed. For new studies approved on or after March 12, 2018, Sterling IRB approved informed consent forms will no longer include IRB review/revision date information in the header. The “SPONSOR VERSION” information and the most recent “DATE APPROVED BY STERLING IRB” will be contained in the footer of the document. A history of revision dates (previously listed as “DATE REVISED”) will no longer appear on the document.
