Effective April 7, 2012, Sterling IRB will require the submission of the Change in Site Information Form for any change in sub-investigator(s) for a previously approved Investigator/Site. In addition, the Change in Site Information Form will replace the Add or Change Site Submission Application.
All requests for IRB review of changes in site information (i.e. change in site name, phone number, site contact person, site address) may be submitted on the Change in Site Information Form. The new form will be available in the Forms section of the Sterling IRB website. Please note: In general, if an individual is directly involved in the performance of procedures required by the protocol, and the collection of data, this person should be considered a sub-investigator.
